The most fundamental piece of information that everybody needs to know when you are dealing with an injury or some ache or pain, it’s limiting your life. what we want to do is help people to understand what this most fundamental first step is when it comes to maximizing your outcome maximizing your recovery with these extra pains and injuries, so how aches and pains need to be dealt with certain secrets or information. Physical therapy treatment or other forms of treatment can have a great deal of limitation. A root cause the way that’s looked at in society today is a diagnosis that’s kind of what people on your stand of a root cause.
How different A Diagnosis and Root Cause?
Diagnosis is usually a specific body part and a specific pathological name such as arthritis of your knee or a rotator cuff tear in your shoulder or bursitis in your hip it’s essentially taking a structure of the body and labeling it with a certain type of problem that is what a diagnosis is, in our current medical system now we have got
Three ways that diagnosis is achieved
1. Parctitioners kind of gut feeling their belief of what they think.
2. Physical tests there are certain tests that we can do, that are designed to try to elicit pain from a certain structure, so for example if we through you had a rotator cuff tear in your shoulder we might do a few different tests that we think put stress on that structure and if it’s painful then that kind of leads us to believe that, that might be where the pain is coming from.
3. Picture X-rays and MRIs and things like that well here’s the problem with all three of those options number on a gut feeling is never a good way to come to a definitive diagnosis because every practitioner is going to have a different opinion and so to be consistent and to be sure that we know where people’s pain is coming from my gut feeling versus that person’s gut feeling we know through research that it’s too diverse there’s too many varying opinions about where pain is coming from, it’s not a reliable valid way to go number two the Physical tests that are designed to diagnose a certain structure.
Research has proven over the last number of years that those tests are not very accurate and the third option which been considered the gold standard an MRI or an x-ray depending upon your type of problem those have been looked at as the gold standard of how we come to a diagnosis about where somebody’s pain is coming from well here is the problem with MRIs and x-rays they two are not able to diagnose the real root cause. All pain-free people are going to have a bulging disc a degenerative disc arthritis stenosis something their lower back is going to have some quote problem in it even though they are pain-free.
So what does the mean to the individual who actually has back pain or sciatica and they get an MRI that shows one of those things well it means that the MRIs might be pointing out the root cause of your problem but it also might not, it also might be picking up something that’s totally benign something that is not your root cause of where your pain is coming from the other fascinating thing that they have done and studied is they take people who are having back pain they give them all an MRI and now they take doctors and say look at the picture and now we want you to make prediction about what people are feeling and experiencing what impact is this problem having on their life and they look at all these pictures these MRIs and they make a prediction about what people are feeling and then we’re able to actually ask these people, okay so here’s your MRI here’s what we think is going on tell me your story what’s going on where’s your pain, how is it behaving what makes it better,
What makes it worse, do you have pain in your back you you have sciatica is a worse when you sit when you stand we walk how does it behave and what we found is that by looking at a picture you cannot predict what people are actually going to feel it’s very inaccurate make that prediction just looking at the picture and then making a prediction of that person, so what does that mean again it mean that when you get a picture taken of your spine or your shoulder or anywhere else and it shows something you have to take it with a big grain of salt because if you don’t if you completely trust what that picture says and you run to the bank and say okay here it is the MRI told me that i have got this and now your mind is fixated on that you could be chasing a rabbit hole that has a dead. some People get an x-ray or an Mri it’s so common to hear people say things like well they said have this really degenerative disc in my back or i have got this arthritis,
They now have this belief that there’s nothing somebody can do for them or they believe that they are going to get benefit from treatment,but there’s no way they can get all the way better because how in the world could anybody get rid of what the doctor has told me i have on my MRI, so we can create this self-limiting belief that doesn’t need to be there the studies tell us that even if you have pathology on an x-ray or an MRI that is is still totally possible to achieve a pain-free state, so that is the first message right there is understanding there are three ways of achieving the diagnosis Expert opinion or gut feeling, physical test and x-rays or MRIs images. Basically all three of those ways have massive limitations in understanding root cause.
How we Achieve the Root Cause and why it Different than a diagnosis?
Getting to the root cause this was something that was described and explained by a guy name Dr.Ron Danielson he was essentially a orthopedic spine surgeon who stumbled upon this treatment method what’s called mechanical diagnosis in therapy and he was sending patients to a therapist was certified in doing this and he was getting results that he was not expecting to get with his patients and he started to inquire about what is this therapist doing that is different why are they getting outcomes when i am not expecting them to get good outcomes and he wrote a book that highly recommend it’s called rapidly reversible low back pain and he actually wrote two books the other is called solving the mystery and that lends a little bit shorter a little simpler read but they’re both fantastic and they explain this process of essentially what is root cause and how do we get to root cause. First big problem when it comes to diagnosing a root cause in healthcare is there’s a big breakdown.
we have talked about we have got MRI opinion and then tests physical test those are our three options to go from here that’s the traditional model and that is the model that’s failing and it’s if we look at pain in society today is getting more and more problematic meaning more and more people are having pain more and more people are having chronic pain or recurrent episodes of the same problem and so the cost to treat injuries is exponentially increasing over time because there is a partly because there is a big breakdown and how these things are being diagnosed.
So people go to get treatment and maybe treatment helps for a short period of time but then the problem comes back or maybe you try various treatments and nothing works if you’ve had treatment and you get a lot better and it stayed better, but as a whole society if we don’t change the system if it will change the nature of healthcare then this process is going to continue to break down. Revolutionizing healthcare is this concept a mechanical exam where we listen specifically to people symptomatic responses meaning how their behaves with certain movement tests that are done,
So we don’t do much care about what the MRI looks like initially we are more interested in when take a certain body part and move it in a certain way do certain tests which is called mechanical diagnosis and therapy it’s the most widely researched form of assessment and treatment for the spine and it’s emerging for the extremities as well and it’s a completely different type of exam and what they are finding certainly in the spine is that people that go through that exam are able to determine within one three sessions who is able to get better who is not.
Do you need another form of treatment such as surgery and what the best treatment is to help that person get better. so we need to use mechanical diagnosis and therapy and that’s going to improve our understanding of people’s problems and then can decide in treatment. The other thing that’s important to recognize is this when go into the treatment like if you were to see a certain practitioner that specializes in the soft tissues and that’s what they do they work on soft tissues that’s there thing or you go to somebody who adjusts the spotting where you go to somebody who really is good at exercise what to do you think you are to get when you go in to see that person you are going to get that treatment because that’s what they do that’s what they like and so what ends up happening in society is people go and they try that and maybe it works for a short period of time, maybe it doesn’t work at all and maybe you get lucky and it works and it fixes the problem and it never come back,
But in research that is not happening often enough that’s why people are having more and more complaints of pain over the year is because problems aren’t solved they are being managed and that’s one of the most common misconceptions is that people are feeling better but they are not actually getting better and lots of things this can make you feel better for a while a pain pill is perfect example a pain pill might help you feel better for a while but it making you better two very different things and so that’s another big part of the problem here is a clinicians specialize in a certain type of treatment where we really need to be focusing on specializing and that is mechanical diagnosis and therapy. The other key is that we have to make sure that we don’t specialize in one thing we need to learn some joint manipulation not every shoe fits every patient,
There might be a couple of different treatment options that are appropriate for that type of problem, use a couple different tools to really make sure that not missing the boat and that really getting the treatment that the person need. So that concept of mechanical diagnosis this process of establishing an assessment and then a diagnosis and then treatment is the number one most failed process that we have in managing pain, healthcare is shifting over the last couple of decades the research is focusing us to took at this process differently so it’s something to consider attempting to contemplate.
